具體要求：提供參加本次采購(gòu)活動(dòng)前3年內(nèi)在經(jīng)營(yíng)活動(dòng)中沒有重大違法記錄的書面聲明（格式文件詳見響應(yīng)文件范本）； （4）具有良好的商業(yè)信譽(yù)和健全的財(cái)務(wù)會(huì)計(jì)制度： 具體要求：提供2020年度會(huì)計(jì)師事務(wù)所出具的審計(jì)報(bào)告復(fù)印件（或掃描件），審計(jì)報(bào)告包括企業(yè)提供的財(cái)務(wù)報(bào)表（含資產(chǎn)負(fù)債表）、利潤(rùn)表（或利潤(rùn)及利潤(rùn)分配表）、現(xiàn)金流量表和財(cái)務(wù)報(bào)表附注，或銀行出具的資信證明復(fù)印件（或掃描件）。審計(jì)報(bào)告應(yīng)該有會(huì)計(jì)師事務(wù)所單位章和注冊(cè)會(huì)計(jì)師的執(zhí)業(yè)專用章，并附會(huì)計(jì)師事務(wù)所的營(yíng)業(yè)執(zhí)照及執(zhí)業(yè)證書復(fù)印件（或掃描件），成立不滿一年的企業(yè)，提供銀行出具的的資信證明復(fù)印件（或掃描件）； （'Calibri','sans-serif';color:black\">5）具有依法繳納稅收的良好記錄： 具體要求：依法繳納稅收有效憑證指：提供2021年6月至今任意3個(gè)月的征稅機(jī)關(guān)出具加蓋公章的完稅證明文件或自主電子繳稅銀行收款憑證，零申報(bào)的只需提供零申報(bào)證明材料加蓋單位公章。 （6）法律、行政法規(guī)規(guī)定的其他條件：供應(yīng)商須承諾：在“信用中國(guó)”網(wǎng)站 、中國(guó) 等渠道查詢中未被列入失信被執(zhí)行人名單、重大稅收違法案件當(dāng)事人名單、政府采購(gòu)嚴(yán)重違法失信行為記錄名單中，并提供相應(yīng)網(wǎng)站截圖，提供不全作無(wú)效標(biāo)處理。如被列入失信被執(zhí)行人、重大稅收違法案件當(dāng)事人名單、政府采購(gòu)嚴(yán)重違法失信行為記錄名單中的供應(yīng)商取消其投標(biāo)資格，并承擔(dān)由此造成的一切法律責(zé)任及后果。
