一、資格要求：1.投標(biāo)人具有合法有效的企業(yè)法人營業(yè)執(zhí)照、稅務(wù)登記證及組織機(jī)構(gòu)代碼證或證照合一的營業(yè)執(zhí)照，投標(biāo)時(shí)需提供原件掃描件（原件備查）。 投標(biāo)人為事業(yè)單位的，應(yīng)具有合法有效的事業(yè)單位法人證書。投標(biāo)時(shí)需提供原件掃描件（原件備查）。投標(biāo)人為分公司的，應(yīng)具有合法有效的營業(yè)執(zhí)照和上級(jí)法人單位授權(quán)書，分公司與上級(jí)法人單位只可一家參與投標(biāo)，同時(shí)參與投標(biāo)的，投標(biāo)均無效。投標(biāo)時(shí)需提供原件掃描件（原件備查）。如果投標(biāo)人為境外注冊(cè)公司，需提供有效的公司登記注冊(cè)證明。2.投標(biāo)人應(yīng)為本次投標(biāo)產(chǎn)品（柔性管）的制造商。本次招標(biāo)不接受代理商、貿(mào)易商投標(biāo)。本次接受所投貨物的制造商全資的銷售公司或控股的銷售公司，以及接受所投貨物制造商的母公司參與投標(biāo)，此類公司視同為制造商，投標(biāo)時(shí)需提供相應(yīng)的證明文件。同一制造商僅允許一家投標(biāo)人參與投標(biāo)，否則相關(guān)的所有投標(biāo)將被否決。3.投標(biāo)人所投產(chǎn)品的制造商須具備有效的GB/T19001（ISO9001）質(zhì)量體系認(rèn)證證書或API Q1證書，同時(shí)需要提交柔性管兩端法蘭配套供應(yīng)商的清單及有效的GB/T19001、ISO9001或者API Q1的質(zhì)量管理體系證書。并可在中國國家認(rèn)證認(rèn)可監(jiān)督管理委員會(huì)網(wǎng)站(http://www.cnca.gov.cn/)核實(shí)。如果有國家相關(guān)部門發(fā)布的最新體系標(biāo)準(zhǔn)，以最新體系標(biāo)準(zhǔn)為準(zhǔn)。投標(biāo)時(shí)需提供原件掃描件（原件備查）。注：如所投貨物的制造商提供的質(zhì)量管理體系認(rèn)證證書是國外第三方認(rèn)證機(jī)構(gòu)頒發(fā)的，則網(wǎng)站查詢不適用。二、業(yè)績要求：（開標(biāo)時(shí)需進(jìn)行信息公開）：2020年1月1日至投標(biāo)截止日（以合同簽署時(shí)間為準(zhǔn)），投標(biāo)人所投柔性管產(chǎn)品的制造商應(yīng)具有至少1個(gè)合同的柔性管產(chǎn)品供貨業(yè)績，且單根柔性管的尺寸和壓力均不低于12寸和4300kPaG同時(shí)符合API 17K標(biāo)準(zhǔn)（尺寸、壓力、API17k標(biāo)準(zhǔn)需要同時(shí)滿足）。投標(biāo)人須按規(guī)定格式提交業(yè)績表，并提交相關(guān)業(yè)績證明文件。業(yè)績證明文件至少包括：1）合同掃描件和2）到貨驗(yàn)收材料。投標(biāo)人所提交的業(yè)績證明文件必須至少體現(xiàn)以下內(nèi)容：合同簽署時(shí)間、合同簽署頁（應(yīng)有雙方簽署或蓋章）、制造商名稱、貨物名稱、尺寸、壓力、API 17K標(biāo)準(zhǔn)要求及到貨驗(yàn)收材料（至少包含：貨物驗(yàn)收單或結(jié)算清單或?qū)?yīng)的發(fā)票（如有清單須提供，須體現(xiàn)發(fā)票號(hào)；發(fā)票需包含合同號(hào)或訂單號(hào)或貨物名稱或買方賣方信息，且與合同或訂單一致）或買方接收證明或調(diào)試驗(yàn)收證明。貨物驗(yàn)收單或結(jié)算清單至少應(yīng)有合同甲方簽字或蓋章。若合同為協(xié)議，除提供協(xié)議文本外，還應(yīng)至少提供該協(xié)議對(duì)應(yīng)的1個(gè)已完成訂單的訂單頁和相應(yīng)的到貨驗(yàn)收材料。同一個(gè)年協(xié)合同提供1個(gè)及以上訂單的均算為1個(gè)有效業(yè)績。 未提交業(yè)績證明文件，或所提供的業(yè)績證明文件無法認(rèn)定滿足上述業(yè)績要求的，均視為無效業(yè)績。Performance requirements(Performance track record will be released to all bidders during bid opening): The manufacturer of the flexible pipes being bid must have at least one supply contract for flexible pipe products between January 1, 2020 and the bid submission deadline (based on contract signing date), with each individual flexible pipe meeting all of the following minimum specifications: 12-inch diameter, 4300 kPaG pressure rating, and full compliance with API 17K standards (all three requirements - size, pressure, and API 17K compliance - must be simultaneously satisfied).Bidders must submit the performance record in the prescribed format along with supporting documents, which shall include at minimum: (1) scanned contract copies showing the execution date, signed/sealed contract pages, manufacturer name, product description, specifications (12-inch diameter, 4300 kPaG pressure), and API 17K compliance; and (2) delivery acceptance proof in the form of either goods acceptance notes (with Party A's signature/seal), payment statements (with invoice numbers if applicable), corresponding invoices (showing contract/order numbers, product details, or buyer/seller information consistent with the contract), buyer's receipt /confirm/iations, or commissioning acceptance certificates. All submitted documents must be verifiable and meet these specified requirements to be considered valid.For framework agreements, bidders must submit both the agreement text and at least one completed purchase order page under said agreement along with corresponding delivery acceptance documentation. Multiple purchase orders under the same annual framework agreement will be counted as a single valid performance record.Any failure to submit the required performance documentation, or submission of documentation that fails to conclusively demonstrate compliance with the above requirements, will result in the performance being deemed invalid.
